Long-term decline of renal function in patients with resistant and non-resistant arterial hypertension

چکیده

Abstract Background The negative effect of arterial hypertension on renal function is well known. Until present, the detrimental resistant (RAH) kidneys was not clearly defined. Purpose To assess deterioration in patients with RAH comparison to non-resistant (AH) during a long term follow-up. Methods We retrospectively reviewed records 307 followed between May 2007 and January 2019 an Hypertension Excellence Centre tertiary hospital. In all we assessed basic demographic characteristics, co-morbidities, laboratory findings, secondary causes AH, medication, blood pressure including ambulatory monitoring, studies invasive procedures (CT, MRI, coronarography). statistically evaluated differences both groups compared decline function. Results 160 had 147 hypertension. Patients were for mean 6 years. older (58.1±10.8 years vs. 51±13 years), more frequently diabetics (39.3% 8.7%), frequent exposure diagnostic (86,5% 64%). Age at baseline visit diabetes mellitus established as significant independent risk factors. Linear mixed model adjusted age demonstrated that have increased than AH: AH increase creatinine +0.24 μmol/l per year, while + 2.12 year (P=0.005). Similarly decrease eGFR −0.66 −1.5 mL/s/1.73 m2 Figures 1 2 show difference slope (based linear models)). Conclusion faster independently diabetes. Further are needed determine key factors affecting subgroups. Funding Acknowledgement Type funding sources: Public grant(s) – National budget only. Renal responses to long-term carotid baroreflex activation therapy in patients with drug-resistant hypertension.

Carotid baroreflex activation has been demonstrated to provide enduring reductions in arterial blood pressure. The aim of this study was to investigate the effect of long-term therapy on renal function. A total of 322 patients were enrolled in the Rheos Pivotal Trial. Group 1 consisted of 236 patients who started baroreflex activation therapy 1 month after device implantation, whereas in the 86...

Renal hemodynamics and renal function after catheter-based renal sympathetic denervation in patients with resistant hypertension.

Increased renal resistive index and urinary albumin excretion are markers of hypertensive end-organ damage and renal vasoconstriction involving increased sympathetic activity. Catheter-based sympathetic renal denervation (RD) offers a new approach to reduce renal sympathetic activity and blood pressure in resistant hypertension. The influence of RD on renal hemodynamics, renal function, and uri...

Long-term quality of life and clinical outcomes in patients with resistant hypertension treated with renal denervation

INTRODUCTION Pharmacological treatment combined with lifestyle modifications is an effective treatment for arterial hypertension. However, there are still patients who do not respond to standard treatments. Patients with pharmacologically resistant hypertension may benefit from renal denervation (RDN). AIM To assess long-term quality of life (QoL) after RDN and effectiveness in reduction of b...
